Appointments

Please print and complete the allergy evaluation forms below and bring them to your appointment.  These forms are required to better evaluate you and learn what has brought you to our office.  Also, bring your current insurance card and a driver’s license or picture identification.

Your evaluation and care are of our utmost importance.  For this reason, we see patients by appointment only and schedule only a very few (8 – 14) patients per day.  As a result, we would appreciate at least a 48 hour notice of cancelation or you will be charged $50 for a late cancellation staffing fee or a no show fee if you do not call.

We require that you pay your deductible (to a maximum of $300), your co-pay and fees for services not covered by your insurance at the time of your appointment.  We accept cash, check and all major credit cards.  If you are not prepared to pay, your appointment must be rescheduled.
